i know it's a tech which can transfer voice also vidio and the signal muct be digital , and if we will use it through phone it should be IP phone , i would like to know how can we design it ? how can we make it exactly ? i know some about it

you can use a standard microphone / earphone headset when using VoIP or even use a stand microphone with PC speakers.
i regularly (1/week for the last year) have a VoIP conference with friends in both houses chatting away through open mike / speakers with webcams.
if you want to build a dedicated VoIP handset (they are available for £10+) you simply need to design a standard USB keypad, microphone and speaker combined into one unit. (the USB protocol was designed to daisy chain items together so combining 3 into one should be no problem)
there are many programs you can download for free* VoIP conversations, skype is a good choice that can also ring landline telephones at a small additional charge.
(*Free calls assumes that you are not paying a /minute connection fee for your internet usage.)
