Converting a PSTN call comming in from modem into voip?

Is it possible to convert a normal PSTN call that's incoming thru a standard 56k internal modem into a voip call that can be answered on another computer across a LAN?

If it is, what software would I require? If it's not, what hardware would I require? Also this setup is required ONLY for LAN, I don't intend to use the voip across the net. Just receiving the incomming call and routing it to another computer.
